Medical consent form?
My dh and I are going to foxwoods over night.
I was reading that I should have a medical consent form that states that either my mom or sister can get Megan medical care (god forbid) something happens.
I called my peds office and they dont have the form. Does anyone have one or know where I can get one? They told me to just write a letter but would that be enough?? TIA

At my ped's office, I just asked about this again the other day, since MIL watches DD during the week and I want her to have the ability to take her if necessary.
My office said just do a letter with DD's name, SS #, DOB, and then MIL's name, saying she's allowed to take her in for care. If I want them to keep it on file, it has to be notarized. Otherwise, I can just have MIL keep one on her and use it if she needs it.
However, at my ped's office, they will not administer any shots if it's not a parent w/the child (which I think is a good thing).
