Medical Consent Form Q
My parents & DH's parents watch our children during the week while we're working/even spending the night at my parents.
Recently I thought we need to have a medical release form, just in case something would happen (they are both at least an hour away from us).
I found a sample form online, however, I wanted to know IF they need to be notarized?
Will this paper hold up at a hospital/doctor's office?

i went and gave it to my doc's office myself to put in my son's chart when we went out of the country. I think, for them at least, that was enough. Call & ask what their policy is.

Thanks....I guess I should have clarified, this would be more for a PM Pediatrics type of place (our regular pediatrician has our signatures & asked who can bring them in).
