Nutty Caramel from Friendlys with hot fudge.
Chocolate chip cookie dough with hot fudge.
Both with whipped cream.

Depends where I go!! 
Carvel: Chocolate soft serve with marshmallow and toated coconut
Cold Stone: Banana and Sweet Cream ice cream mixed with caramel, graham crackers and fluff
Baskin Robbins: depends on what flavors they have, usually just plain!
Friendlys: Peanutbutter cup Sundae with pistachio ice cream
