NYC public school teachers - paperwork question

What forms did you fill out and when?
Before I submit everything, I want to make sure that I have the correct dates, etc.
My payroll secretary wanted me to hand everything in before I left, but an acquaintance who works for the UFT told me not to, she said that I don't hand anything in except the long form with the boxes until after I've had the baby.

I'm about to send the FMLA form in. I plan on taking off until September. But do I just fill this initial form out for the 8 weeks entitled to the C-section and then fill out a new one, along with the leave of absence without pay form, for the rest of the 12 FMLA weeks, and then another one for the rest of the year?

Any help would be greatly appreciated :)

I'm not 100% sure...but I think I only filled out the leave of absence form ad my payroll secretary filled in the dates for me.
