Pediatrician's Office-Charging to have forms filled out

Does your office charge to have physicals filled out? if so, how much? and do they charge it on top of the visit and if so do they charge you per form?

I'm very curious about this and how other ped offices do this.. Thanks!

at seaford peds if you use their generic form it was free, $5 for other forms and you have to pay at the visit...or it may have just been $5 for either, can't remember

So I made an appt for my 3 kids for well baby checkups.

2 are going to camp and starting a new school in September so I have forms for the dr. to fill out.

So, I'm getting charged $20/copay for each kid and they are charging me an additional $15 PER FORM.. which is an additional $30 bucks PER CHILD.. Am I wrong for questioning this? This doesn't include the fact that I know I'll be getting a bill from them for my deductible as well..

Our office has a package where you pay $40/child per year, max of $100/family. Unlimited forms and copies of immunization records, they will mail or fax them (or both), they will also call in prescriptions (like a renewal on their vitamins).

Otherwise we can get one copy of their immunization record per year and $20 per form, have to pick up prescriptions in the office.

I worked at a pediatrician's office during college and I don't recall the exact amount, but yes, we charged. It was pretty cheap, if I recall (definitely not $20 per form). The reason being is that around this time of year (camp) and before the start of school, we were completely inundated with forms. It could literally be someone's job to sit there and fill them out all day (in fact, this was kind of was my job) and it took time away from other jobs that had to get done, like insurance billing. So basically we were billing for our time.

I totally get that and I'm not arguing having to pay a fee but I think that $15 PER form is excessive

I agree, that is excessive. I think we charged a couple of dollars. That was years ago, but I'm sure it's not that much now.

$15 is ridiculous!

They got very snotty about it, said they don't do that. I ultimately switched practices. Sorry, but if you are charging me $5.00 for 5 minutes of your time (not even), you can take another 30 seconds to fax it, or drop it in your outgoing mail.

Yeah, I don't get that either! It's sad that all they try to do is get your money one way or another..

It's more common practice now. My friend was just telling me about her office. 15 seems excessive. I need to call my peds office tomorrow to have them fill DD's out I'm wondering if they implemented a fee since the last form I had!

OR

$40 for the year. For us this is the only option. We need TONS of forms for DS.

Our office charges $15 per child each year. For that fee that will fill out unlimited forms.

i thought they weren't allowed to charge a copay for a well visit?

just this year my office started a new policy - an annual fee of I believe $50 for forms filled out....my DD is only 1 year old and needs no forms filled out - I was annoyed but had to pay anyway.

I have never heard of this with any doctor I've ever been to. I guess I can see where they are coming from with charging a fee for their time, although I do kinda agree with PP who said it is really part of the job as office staff. Either way, $15 per form per child is definitely excessive IMO!!
