I would say Island Therapies. My son went there for OT and they were suppose to be the "best" on long island for that.
I used south shore speech for all of my evals. I was very happy with them. They really specialize in speech though. Although I'm sure they do PT evals.

I know Just Kids in Middle Island and Riverhead does evaluations.

We used Metro Therapy. The PT who came (Tom) was so nice, and gentle with Erin and really seemed to care about her, and at the same time was such a cheerleader for her. We start therapy the week of March 24th with a different PT... Tom lives too far to do the 2x a week PT but was able to come for the eval.
