Question about IVF and day of transfer
So we just had a consult which ended on kind of a bad note because we discussed the difference between implanting 2 on day 3 or 1 on day 5 and were told by the Dr we would have to decide when our cycle started what we wanted to do. They would NOT call us on day 3 and say the embryos look good enough to hang on until day 5 or not.
My question: Is this usual? Can people share the name of a clinic where they did NOT do this? TIA!

I'm with RMA of NJ, but in our IVF consult my RE told depending on how many eggs were retreived and fertilized, and due to my age, on a day 3 transfer they would put back 2-3 and a day 5 transfer 2. My nurse told me that they typically do a 3 day transfer. I will get a phone call that day to let me know if I have to come in for my transfer or what time to come in if they wait to do a 5 day transfer. For personal reasons, we are hoping for a 5 day transfer.
ETS: We also don't have to decide at the start of the cycle. Chances are though that we will follow the doctors recommendation

Never heard of that....was that at New Hope???
As you know I am at Cornell and you don't decide in the beginning...it all depends how many embryos you have after fertilization...based on that they monitor them and let you know on day 3 if you will come in for day 3 or wait for day 5......
ETA: They also told me that they will only put back 2 based on my age and the fact there is nothing wrong with me.....

at LI IVF they typically do day 3 transfers. The RE told me that on a rare occasion they will do a day 5 transfer, but that is only when they feel there is no chance that the embryos will not survive. I think Cornell does a lot more day 5 transfers. But to answer your question I have never heard of deciding b4 you do the cycle. i would think it matters what the embryo quality is and you wouldn't know that until after fertilization. A lot of clinics don't want to take the risk of an embryo not making it to day 5. HTH

I've done IVF at 2 different clinics, one in St. Louis and one here (LI IVF) and I have never heard of having to decide before your cycle starts. How can you possibly decide now? You don't know how many eggs you'll have, how many will fertilize and how many will make it. At my last clinic, they would tell you the day after retrieval how things look and if they think you will make it to day 5. If they think you will make it to day 5, you have to be on call every day because sometimes on day 4, they'll tell you to come in if things start to look bad. I think the procedure is the same at LI IVF.

this was at Li IVF
that was not my experience at LI IVF. From the beginning I was scheduled for a day 3 transfer. I produced 19 eggs and 17 fertilized and I still did a day 3 transfer. I have yet to hear of a case at LI IVF where they did a day 5 transfer. Some clinics just like day 3 better.

Are you going to do a natural cycle or a medicated one? also can you decide as you are going along and can kind of predict how many eggs they may retrieve?
I ask because one if you do a natura ivf you have a lot lower chance of having a fertilized embryo making it to day 5. and second if you do a medicated ivf and you see that you have a lot of follies growing than it might be easier to make that decision. Also on my last few ultra sounds they said I had 30 follies and ended up with 19 eggs, still a good number but a pretty big discrepancy.

I never asked them if day 3 vs day 5 made a difference in # of transfers. I guess at my age I'm just hoping to have enough to fertilize and transfer in. Actually, they haven't even talked to me yet about the number they plan to implant. I should be taking notes, I can never remember when I meet with my RE (which is usually never).

That seems completely riduclous to have to decide before you cycle starts! I can see them wanting to know the day after your retrieval once you get the fert report, but for me THEY told me on day 3 they were pushing it to day 5 since everything looked great. At my clinic they choose to do day 5 transfers if at all possible though, which I know is different then LI-IVF. I would tell them day 3 to start since you don't know how you are going to respond to the meds and then when you get closer and if it looks like you will have a lot tell them you want to switch to day 5. Remember, YOU are paying them a LOT of money to do this so you should be in control of what happens. Good luck

I cycled at LIIVF and I just pulled my paperwork. I keep everything...You have to fill out the consent before the cycle, which does state if you want a Day 3 or Day 5 transfer. But in the upper left hand corner, there are boxes to check for "override" by the lab, and one of those is day of transfer, as well as ICSI, etc. Of course, I asked like 50 questions. It was explained to me that if I chose Day 5 for example and I didn't meet criteria, the lab could "override" back to day 3 after discussing with me without having to sign a new consent. Or they can switch to day 5 if my embryos look great withtout having to again sign a new consent. Basically, that 90% of their transfers are Day3, transfer 2 and the risk of multiples was pretty low, they quoted a pregnancy rate of 50% and of the people who got pregnant, 25% had more than one with 95% being twins, so your risk of twins with 2 back is really only like 12% . I would have been ok with twins, I figured buy one get one free ( haha). But luckily have two from separate cycles. They do Day5 for PGD, hyperstim pts sometime and couples who absolutely want to avoid twins. They have a criteria for Day 5 transfers, in my notes, Dr. K said it was at least 8 fertilize, under 40 years of age and at least good development on Day 2 ( he said they don't decide on Day 3 for scheduling reasons, so they can decide the day before and give you enough time to reschedule work or whatever.) I opted not for Day 5 because I didn't want to chance my "extra" embryos not being frozen. Did you talk to a nurse or the doctor? I know Dr. Kenigsberg would let you do what you are asking.

Thanks for all the info. The more I think about it the more I think my husband pizzed her off and she didn't want to deal with him EVERYONE is saying the opposite of what I was told. I talked to a different Dr-not Dr. K

Thats weird.....We decided once retrieval was done and the embroy quality was seen...
Our embryops werent "making it" ..so we rushed ( in a sense) to do the 3 day transfer as out of 15 eggs , only 5 made it, and 3 were icky quality.

ditto for Columbia also. My RE told us that they would monitor my embyos and see how they do. We transferred back 2 5 day blasts. He did however state that just because they made it to 5 day transfer doesnt mean that if they did a 3 day transfrer the result wouldn't be the same. Some embryos do better growing in their natural environment and no matter how good technology was nothing could exactly replicate a uterus.
On a side note we had 28 eggs retrieved 18 fertilized 7 (I believe) made it to blasts and we had 2 transferred. One implanted (DD) and nothing leave to freeze. The others in my RE's word died off not strong enough which s u c k s because if DH's varicocele surgery doesn't work we have to start another IVF from scratch. I wonder if we would have done a 3 day transfer if we would have had some for FET in the future.

mmmm where to start, well ive been told that the transfer is either on day 3 or on day 5 this is determined with the egg is fertilised and how well it is growing, if growing well it goes back on day 3 if the egg needs longer to grow it will be put back on day 5. Only ONE egg is transferred unless you are around the 40 year old mark where then they will put back in 2 for a more success rate and because you are getting a little older and the success rate when you are older is more rare. When I signed the consent forms at the very beginning of the proceedure you then are told whether one or two eggs will be transferred. Dont forget if 2 eggs are transferred and they take ie twins there is a greater risk within the pregnancy.

That's not true. I know how many eggs are going to be transferred on Day 3 and Day 5. I'm not around the 40yr old mark either. Other factors besides age are taken into consideration.
