Question about mixing powdered formula

We just started the Neocate today since the doctor told us we could use up the Alimentum we'd already bought. We put in the 5 scoops of powder and then filled to the 5oz line with water, but when we shook the bottle up it was only 4oz. We added more water since I wasn't sure what to do and didn't want it to be too thick, but what was the right thing to do?

you might want to put the water first the powder takes up space on the bottom
