question about paperwork for FMLA & disability
i got all the paperwork that needs to be filled out for the FMLA and disability. I had to fill out some and then i have to give it to my dr. to fill out then it has to be given back to my job..
did any of you do this paperwork yet? I dont know if i should bring it with me to the dr. this week, but they probably wouldn't fill it out right away...
what did you guys do?

Funny you asked. I asked my doctor about it last week at my last appointment and she told me I could bring it any time.
The only issue with me is that I am working until the day the baby comes, so they won't be able to put a date on the paperwork.

just be careful, disability wont accept pre-dated materials (they denied my claim because the paperwork was filled out too early)... I had to fill out additional papers after ds was born plus write an explanation... I would just wait until you have the baby...

oh really?? actually fill it out afterwards? I didn't think about that..but then it would take that much longer to get the disability, right?

oh really?? actually fill it out afterwards? I didn't think about that..but then it would take that much longer to get the disability, right?
yeah but it took me even longer cause I went to the doctor 6 weeks pp and she filled out the papers then... but after that I got my check within 1 week... I think what got them confused is that they thought I wanted to get paid for the couple of weeks before I had my son (I stopped working at the end of May and had my ds in the middle of June)

I had my paperwork filled out about 2-3 months ago (I'm now at 38.5 weeks). My HR said that it needed to be submitted 30 days before your projected date of birth. I brought it to my appt. and teh Dr. filled it out and mailed it back to me about 2 weeks later. And they charged me for filling it out!

same with me I couldnt even fill everything out cause I didnt have the baby yet and dont know when he/she is comming my midwife said just to bring it to every apt till we get to the point where we feel that I should no longer work So I will bring it with me to my next apt then the next but do plan on stop working on the 18th if the baby hasnt come yet
