Short Term Disability
As I may have mentioned before my HR rep at my company is not the brightest bulb of the lot...anyway, I asked her about the short-term disability forms and I got, "I don't think we need to fill anything out, I mean so-and-so (another girl who went out on maternity leave) didn't fill out any forms."
I was told by several other people that your employer does fill out the STD papers for you? Does anyone have any information on this?

I think if you are using NYS disability that your doctor fills out the forms, your office doesn't do much.
If they have a private policy, then maybe it is different, but I am not sure.

One of the Girls in my office went out on maternity leave and the dr's office filled out the forms and then mailed it to my boss who i think had to sign and mail it in.

The NYS form has three parts, one for you to complete, one for your doctor to complete and one for your company to complete......you can go to the NYS DOL website and get them, I believe....

I am on STD now. My supervisor filled out a part of the form however I had to fill out an employee statement and authorization for my medical records and have my DR. fill out the attending physician statement which I mailed to the STD provider.
