One of the kids at works moms' has stationary...
Its 3 kids (they have 2 boys and a girl) and then on top it says "A note from (then you check the box with one of the kids name...ex: you'd check Sophias name if the note was re: her or to her teacher) Mom...
I love it! But dont know where you'd get it!

Jen - if you have a color printer, you could just make up your own. That's what I did with my kids.

I've gotten a bunch of note cards from them....for ourselves as well as for baby gifts....nice selection and quality

I am using stationarystudio.com for Roman's notecards. They have a huge selection and great prices too.
