What Insurance do you have if they pay for your formula?
And how does that work? Do you need a prescription or a note from the doc?

we have Emprie gov't. - DH works for the LIRR.
we get the formula (nutramigen) through continued care not the insurance company.
we got a prescription from her pediatrician. they call the dr too.

We had United Healthcare, but the coverage will vary based on what your employer chooses, not all UHC members have the same coverage.
Anyway, they covered Neocate when Andy was on it, we had to get a prescription for it and the ped had to fill out some form and write a letter about his diagnosis. They denied our claim to be reimbursed for Alimentum since it's available OTC, but he was only on that a short time we didn't pursue it. We were just so happy they covered the really expensive formula.

They cover our Nutramigen..thank God!!
The doc wrote out a prescription for a months supply. I brought it to the pharmacy like I would if it was medication. It takes a few days but a delivery of a months worth of formula comes in and thats it. It's so easy and just saves me hundreds a month. But we pay insurance which is HUNDREDS a month, so they better lol

I have GHI and they reimbursed me for the Nutramigen. Doc had to write a note explaining why DS needed it due to his reflux.

We have Caremark and we needed a prescription from the ped and a letter documenting DD's condition in order for the insurance company to pay for 1/2 the price. We get the formula thru the pharmacy in one month supply increments (45 RTF bottles) and are only approved for 6 months. We have to resubmit for more months in order to have it covered and it may be denied.

We have Empire Govt too. We had to put up a huge battle to get them to cover DS's Elecare. It took weeks for them to finally do it. Our Dr called the insurance company everyday. Finally one day I got on the phone with them and cried. I told them that DS had severe allergies and the Elecare is the only formula that worked. I told them that if he doesn't get it, he would end up in the hospital. And then I told them if anything happened to him, I would hold them personally responsible. Within two days of my melt down, we had 2 cases shipped to our door...Our Dr wrote us a prescription good for an entire year.

I have UHC Choice Plus (If that matters) and they covered Neocate with a $20 copay for 90 days worth. They covered it with only the prescription faxed to Medco by the pediatric G.I. It was the easiest thing I have done with insurance! They covered it for 1 year and would have extended it if needed.

We have Magnacare and they DO NOT cover it- although they originally told me they would. We are actually going to a meeting with them next wk to appeal it for a second time.

yes, do you do need a prescription. Here's who I used to contact my insurance: Apria healthcare 1-877-536-8372. You contact them give them your insurance they will say..we will try to get your insurance you cover your formula in the meantime fax over your prescription. Then they will contact you either way, but if you don't hear from them within a week call them. My DS was on similac sensitive after I stoppped BF. Good luck!

BCBS. They paid for almost a year $1400 a month. I just got a letter(he's been off formula for 6 months now) that they would not be paying it anymore because it was approved in error

We have oxford and we had to fight with them to get it covered but eventually we did get it covered.
I still have elecare left over if that is by any chance what you need.
