Some questions about applying for Perm Certifcation

1. I graduated from CW Post in 2004 and they applied for my provisional...Now do I have to go through them for my Permanent too???

2. I also have to change my name before I apply...everything is in my maiden name. How do I do that???

As far as I know, the NEW guidelines went into effect in 2004 so perhaps they endorsed you for your INITIAL certificate? I would double check. I remember when i was in grad. school, I finished my second certification in 2003 and graduated in may 2003. they were adamant in telling me that we were the LAST class to fall under the old guidelines and they encouraged people to finish quickly so they would be under the old guidelines.

Regardless of whether or not it's your initial or provisional, you don't have to go through Post again. the state has you on file. i would go to nysed.gov and apply online for your permanent....that's what i did. it was SO easy... just create an account and you can even pay for it on-line.

Regarding your married name, i applied for my permanent cert. with my new name although the provisional was in my maiden name. apply on-line and then send a letter with a photocopy of your marriage certificate. they will put your married name on the new certificate.

I did a name change on my other certficate that was already permanent. i recently went through the tenure review process and gave them a copy of the certificate with my new name. my district said that it wasn't necessry for me to change to the new name (I spent $25.00 for the name change) because the state tracks you by your social security number, not your name.... but i did do it for the other one i was applying to make permanent because i was already married...

ETA: you can apply for the name change on-line too if you want to change your other certificates to your new name....

It's my permanent....I graduated in 2004 but really had my Masters in Dec of '03 so I got in right before the guidelines were changed

I did everything online as well. There was already a record of the tests that I had passed, and it was very simple.
