Disney and diaper bag
First i used a back pack instead of my normal diaper bag for convience. And aside from regular diaper bag items that you listed I brought things to keep them entertained on long lines ( my DD (4) is needy;). (action figures, books, crayons and post its) some snacks. Have fun!

Disney and diaper bag
I used a backpack as well. I brought normal diaper bag items and some bottles for DS 2. I didn't pack the backpack too big and put a lot of the items in the bottom of the stroller so it would be easy to take on rides.

Re: Disney and diaper bag
I plan on just taking a good backpack. Done it for other trips and so much easier. Get a good one with lots of compartments and comfy this way you can separate everything and have it all easily accessible. I keep a compartment for some diapers and wipes, one for extra clothes in case, snacks, and any little toys. Of course you need for your wallet and stuff as well along with a first aide kit, use the sides for drinks. Oh, and of course the changing pad shoved in there.
My 4 yr old is potty trained, but I like to keep extra underwear for him incase. Mainly stuff for my 2 yr old.

Re: Disney and diaper bag
Same as above - used a backpack - so much easier, plus hubs could wear it too.
In addition to normal diaper bag stuff I'd add suncreen and sun hats.

Re: Disney and diaper bag
I packed a cooler w/ drinks and kept that at the bottom of the stroller
I kept my usual diaper bag stuff in a backpack w/ a lot of snacks .......(but mine are 2 & 4 so it's mainly diapers/wipes/change of clothes for both clorox wipes, hand sanitizer, bandaids, neosporin)
I didn't bring entertainment for them though - it's freakin' Disneyworld!! the place is overstimulating to begin with
