does insurance cover them for each pregnancy? is there a time frame before you can get another?

mine allows one per pregnancy - no time limit. Have to have doctors script for it.

Not all insurance companies cover it...If you have NYPD insurance they def dont, I only know because I called and got denied.

Every insurance is diff. Mine covers one new pump every 36 months. In between the 36 months they cover some replacement parts.

I don't see why you couldn't. How would they know if you already have a pump or not and whether it was through insurance or self pay.
To the OP, every insurance is different. My insurance didn't cover with my last pregnancy so I bought one. With the new laws, they now cover and mine covers one per year per child I believe. I didn't even need a script from my Dr, I just called and they sent me one along with milk storage bags. It's not the greatest pump but it will be nice to have a back up. Not every ins covers them though, some are somehow grandfathered with the laws and aren't forced to cover them.
