Clomid with OB/GYN

Hi ladies! My OB/GYN would like to start me on clomid next cycle (days 5-9). Since I would be doing this with the OB and not RE, it would be unmonitored. We suspect I have weak ovulation/low progesterone as I spot a week before my period every cycle.

DH will need an SA prior to my starting the clomid.

I know everyone says it's better to do clomid with an RE, but I'm thinking it wouldn't hurt to get the ball rolling and get a cycle or 2 in while waiting for an RE appointment. Has anyone ever taken clomid with their OB and had success?

IMHO you are just wasting time doing it with an OB/GYN. I know because I did it.

They don't monitor you which doesn't help in trying to figure out your cycles etc. Testing should be done first before even starting Clomid. Also if are doing an IUI with the clomid there is a real possibility that the timing for an IUI will fall outside of normal office hours and then you either don't get the IUI or you have a poorly timed one and it won't work.

I think you should just make the RE appt. An RE is going to make you go through a litany of tests that will help to determine the protocol. Since you don't know if or what the issue is you are going into the Clomid cycle blind.

First, how long have you been trying?

I would NOT take clomid under the care of an OB/Gyn for a lot of reasons.

For one, you say you "suspect" you have low progesterone because you spot before AF. Well, I don't have low progesterone, and I spot before AF. So really, you don't know if low progesterone is even your issue. Also, clomid can help with a stronger O, but won't necessarily increase your progesterone much AFTER O to help you sustain a pregnancy, if low progesterone is really your issue.

Also, clomid can thin your lining which will obviously not help you get pregnant. Without being closely monitored, you'll have no idea if clomid is actually harming your chances.

And as the PP said, your O could likely fall outside of regular office hours. What then? REs are open 365 days of the year so you wouldn't have to worry about this.

There's no such thing as "getting the ball rolling" while you wait for the RE appt. An RE will require you to get certain tests, and having just been on clomid isn't going to speed up your process with the RE. If you've already been trying for a while, call and make the RE appt. Some REs can get you in right away.

And finally, an RE's job is to help you get pregnant. The OB's job is to help you have a healthy pregnancy and delivery your baby. Let each dr do their job.

exactly what everyone else said. don't waste your time. i did the same and regret it. i know you think it wouldn't hurt before you take clomid while you make a RE appointment, but your better off going to the RE first and having testing done to confirm if you have low progesterone. if you do, clomid will think your lining and may not help you get pregnant because your lining will be too thin. and even if you do get pregnant, you might need the help of progesterone supplements to help keep the pregnancy. Please, PLEASE, seek the attention of a RE before starting anything.

Thanks ladies. We haven't been trying terribly long.....about 9 months (but 5 cycles, because I have LONG cycles, which is also part of the issue) but my OB/GYN agrees that the likelihood of getting pregnant when I start spotting 7 days after ovulation is slim. I've had an ultrasound which showed some tiny cysts and CD3 BW which was all normal. She said we could try clomid or supplemental progesterone, but clomid would be better (I was kind of hoping for the progesterone). She didn't say I wouldn't be monitored, but I can't see how it would be possible.

DH will be tested prior to starting clomid because obviously if something is wrong with him clomid won't help that. Assuming DH is ok, we would not be doing an IUI.

I 100% agree that the RE is the better option, that's why I was wondering if anyone did have success with the OB, because I'm skeptical. I do wonder if an RE would give a hard time being we've been trying less than a year, however, I can't possibly see how I could get pregnant with a 6-7 day luteal phase.

I'm new to all this and it's a little overwhelming!

I spotted for years. Finally complained to my GYN and they did a water sono and found polyps. Had them removed and bam no more spotting.

Spotting doesn't mean a progesterone issue. I would seriously considered a new dr if they are suggesting clomid without more testing.

Now, see, if you were under the care of an RE you'd do Clomid *and* they'd test you at 7 days past ovulation to see if you need supplemental progesterone. It's not one or the other. I had to have both together (and since I was being monitored closely, they knew I needed the progesterone at 3 dpo, actually).

It's not possible for the OB/GYN to monitor you. That's not what they do. Monitoring involves you going in from about CD 7 to see how many follicles are growing and how big they are. They will check you every few days until the follicles are 18 mm or bigger and then test your blood to see if you're surging. This is why your appointments need to be before 8 AM or so - OB/GYN offices aren't even open yet. If you're not surging, you'll get an HCG shot (which also helps support conception/early pregnancy) to make you ovulate and you'll have intercourse or an IUI a day / day and a half later.

I agree with previous posters. I think doing anything fertility related that involves medication with an OB/GYN is a waste of time. You need to be monitored closely on fertility meds for them to be successful. In addition, an OB/GYN's job is to keep mom and baby safe/healthy throughout the pregnancy once you've already conceived. A RE's job is to get you pregnant, which is what you clearly want. Two totally different jobs.

to answer your other question, i don't know of anyone who has taken clomid with an OBGYN and has gotten pregnant. No RE will turn you away because you have "only" been trying for 5 cycles. They are there to help you, not turn you away until you hit a certain criteria

2 things....

An RE will not turn you away if you have only been trying for 5 cycles, especially if you supsect something is wrong.

And your LP is counted from ovulation to full flow, not spotting. So your LP is not 6-7 days unless you're actually getting full flow a week after confirmed ovulation.

Again, an RE will clarify all of this for you and come up with protocol.

Thank you all so much for the responses. You bring up excellent points that I didn't think of so I'm very happy I asked! I'm going to speak with my OB next week and let her know I'm not comfortable taking clomid without monitoring. If it means I wait a couple months, then so be it.

I'd like to go to an RE at Cornell, so if anyone has used a particular MD there I'd gladly take a recommendation!

you are only supposed to take clomid 6 times in your life time. I wouldn't waste 2 on a obgyn.

I did. my DD2 is 5 weeks old.

eta- I did have cd3 bloodwork as well as prog monitoring thru my ob.

I'm doing this now as well, I'm on round 3 of Clomid only going for the 21 day blood test & my husband has been tested as per my OB ( he's fine) I've decided to seek the help of a specialist named Dr. Trivox, I'm 38 yo & have no more time to waste. Depending on how long you have been TTC you should find a specialist.
