How to apply for the temp disability?

I asked my HR rep about the NY temp disability and she told me this (along with the paperwork):

"Attached is the Disability Claim Form. You will need to complete Page 1 (PART A), your physician will complete the top portion of Page 2 (PART B) AFTER you have delivered your baby. When both of these parts are completed, both pages are returned to me, I will complete the lower portion of Page 2 ({PART C) and send it in to our Insurance company.

NOTHING can be done until you have delivered your baby."

Is that true that I shouldn't be doing anything until my baby is born?

I don't entirely trust my HR as she's really just payroll and I feel attacked anytime I ask for ANY general information so I'm just clarifying with any moms that have done this before. Thanks!!

It is true. It kicks in after the baby is born, so it has to be filled out after you give birth.
And there is a one week waiting period where you get nothing.

My Dr's office fills it out ahead of time for me. I brought them my paperwork last week at 36 weeks. They just file an amendment if you go earlier/later. But I have a planned c-section so I expect to go on time (unless DD has other plans!).
