Sounds like OCD. DD does similar things. She doesn't outgrow them, they change though. I would talk to your pediatrician and see what he /she says. OCD is not something they can help but you can redirect the behavior into something less noticeable.

You mentioned he has delayed fine motor skillls. Does he currently see an OT? This way be a good thing to bring up to them as they deal with not only fine motor skills but sensory skills. If your child has difficulty with his hands he may (again I do not know your child at all) be looking to get sensory input from another way.

DS would peel the skin off his thumb - to the point the skin was raw. DH and I noticed it about six months or so ago. We mentioned it to him and encouraged him to stop at the time. I haven't noticed him doing it. Just checked his thumbs and the skin looked normal.

ETA: DS was about 6.5 at the time

My dd had ticks at various times. We did take her to a neuro but they said normal and she would outgrow it and did by i think 11yo?

I would certainly keep my eye but dont draw too much attention to it either. No harm in asking doc of course.

It sounds like OCD.

I have struggled with OCD since I was a very young child. I kept it pretty much hidden (I started telling people only a few months ago), but I wish I hadn't; I could have used help when I was younger because it has been quite obtrusive at points.
