Maternity Leave
When did most of you decide to leave and how far in advance did you have to fill out the STD papers?

I went out about 3 weeks early because I had a few instances of early labor that were really intense. I actually went to the hospital and was sent home. I was terrified the cramps would come on while I was driving to work (over an hour each way) and I'd drive off the road or something.
My doctor filled out my STD form at my 6 week checkup.

I am leaving 2 weeks before my EDD. I'm not eligible for STD so I don't know about the paperwork.
