ETA - I think when I was younger the concept was more foreign to me than in this day and age - today, it's no longer a riddle - it's a no brainer!!!

Good call Goldenrod! It's the day and age of blended families!
I gave my 2 guys that quarter riddle. My older son could NOT get it. He was coming up with complex ways to try and answer it. My 6 year old got it IMMEDIATELY!

This one is so old I knew it before I finished reading the first line.
Even the first time I heard it, years ago, it was pretty obvious.
ETA: The first time I heard it a little differently, there's no mention of the father being in the accident and dying. The surgeon says "That is my son" and the boy wakes up and says "That is not my father", how can that be?
