Each doctor is different, but after a year we went at:
15 months
18 months
21 months
2 year
our next visit is 2.5 years.

here after her 18 month visit....no visits until the well visit before she starts school.
but the nurse said bring her back in whenever i want, they would be more then happy to see her even if shes not sick!
DD gets tons of ear infections so im sure she will be there a few times this winter still 
ETA: she went at a year and then 15 months and 18 months.

same for us except we don't have 21 months. From 18 months we go straight to 2 years.
From age 2-4 I think it's every 6 months. And then it's just once a year.
