Asking here because I need quick help. TIA!
JT and I are going away early tomorrow morning and leaving Ava with my parents. I'd like to leave a letter indicating that my parents have permission to make medical decisions for Ava in the event we are unable to. Does that have to be notarized or can we simply type something up, sign it and leave them a medical card? I don't even know where I'd get that notarized by tomorrow. We're not going far and we're only going for a couple of days, but it just dawned on me that we should do this. Any help would be appreciated!

I have always left a note w all relevant info and signed it, but Ive never gotten it notarized.HTH

whenever SIL leaves our nephew with MIL she just types a short note and her and BIL sign and dtae it

When my cousin went away a few weeks ago I noticed my aunt had this type of medical permission letter on her fridge for their daughter. It was notarized, does it have to be? I don't know, but its always a good idea.
I'm a notary and would do it for you, but we don't live any where near each other

ETS: I did a search online and there are many sample medical consent forms most of them do not have an area for a notary, some for a witness, but almost all of them only ask for the parents signature. So you should be ok...

we did ours last minute and I didnt have time to notarize it, nut I did have 2 coworkers sign as "witnesses" just in case.

Definately don't need it notarized. We get them all the time from patients in our office.

You definitely don't need it notarized...but make sure you do it.
My friend took her nephew to Disney a few years ago and he had a seizure in the hotel. If she hadn't had the note (not notarized) giving her power of medical decisions they were going to hold off on treatment until they contacted his mother (who was in Jamaica).

I have one perm. on file at my docs office cause you just never know. It states if Darren or I are unavailable or incapacitated or whatever...

Thanks everyone! I had no way to get it notarized tonight, so I just typed up the letter and signed it and I'll have JT sign it before we leave. We'll only be a few hours away, so I'm not too concerned, but I'm a worrier and I feel better knowing I wrote the letter, at least. I'll know to get it notarized for next month when we go away again.

It could be used for minor things, but it would not stand legally. Write it up for "peace of mind", but something like that needs to be notarized. Think about it. Someone else could have written it just as well. HTH! Sorry, you asked!

G-d forbid you need it to stand up in court, it must be notarized. Another g-d forbid something happens I don't know if a doctor /hospital would accept a letter without a notary and/or a witnesses signature.
