A Boppy or a My Breast Friend pillow. You may also want to register for a pump if you plan on pumping.

In the beginning...I leaked a ton, so definitely breast pads. I never needed the lanolin, but I think I'm in the minority. Boppy pillow and a few extra covers.

A boppy, extra parts for pump so u can have one being washed while u r using the other, lanolin for ur nips (I actually use/prefer Earth Mama Angel Baby Natiral Nipple Butter.. U can get it on amazon). I also bought/love the medela labeling lids. Medela makes pump cleaning wipes that r great if u r pumping at work or someplace u can't wash pump parts right away
ETA- nursing pads! Washable r more comfy but disposable hold more "leakage.". Also wanna add that if u r going to pump u will def need the lanolin/nipple butter to lube up the pump part that goes on your ninnies

also, register for a cover or drape. it came in very handy for me, esp when i had lots of company in those early days but still needed to nurse.

i dont know if you necessarily want to put this all on your registry, but this is what i needed while BFing: TONS of nursing pads lots of nursing tanks/nursing bras pump, with extra set of breast shields medela microsteam bags breastmilk storage bags cleaning wipes for the pump parts

I too am in the minority and do not need nursing pads, but most people do. Iwould totally recommend the lanolin creams/ointments- i prefer lansinoh but medela also has one and both work. You will need storage containers if you pump. I also still registered for some bottles- mainly medela. Medela carries an accessory package that i also got. Really helped out. I didnt get a cover, and on the one or two occassions i needed it, i used a blanked- light weight, and covered up quickly. The nursing tanks are awesome, not a huge fan of the bras- i am quite large chested and noen fit right. Get a good water bottle- you will need it, a good book or two, boppy pillow. Good luck!!

My Brest Friend pillow. I had the boppy and it really wasn't helpful with breastfeeding, and I have My Brest Friend pillow is sooo much better.
Gel Soothies (you can put it in the fridge and it helps your sore nipples)
Earth Mama Angel Baby Nipple Butter
breast pads because you leak a lot at first
Medela pump I love the Pump in Style

I always used the Boppy. I'd get a pump, even if you don't plan to use it much, as you never know... I was lucky & got a hand-me-down one, never expecting to use it. But, my daughter was in the NICU for a week, so when I came home and she was 2 days old, I needed the pump...thank god I had it already! I didn't need nursing pads, but a lot of ppl do. I cleaned the pump parts with regular dishwashing detergent, but I'd def ask the hospital nurses for freezer bottles... that's all I used. And, be sure to get nursing bras: I use my regular shirts (well, the ones that make it easy to pull out a boob with), but def need nursing bras.

If you plan on pumping at all a hands free bra is great to have as well as extra bottles to pump into or store in fridge.
