The title of your post, as well as your link mentions Empire - but NOWHERE can I find anything official that says Empire.

As the subscriber, we had to complete
Section #1 on page 10
Section #2 on page 11
Section #6 on page 15.
Am I missing something? Where do I find this information? We had UH for a few years at one of DH's jobs.

Click on the orange text:
"Mailed Notice + Proof of Claim Form"
You will see 16 pages of documents.
Go to page 10. Fill out section #1
Go to page 11. Fill out section #2
Go to page 15. Fill out section # 6
Mail in the form

I would try anyway. I have Empire BC/BS and I was sent paperwork in the mail automatically for it.
I don't know if the reps at work are not always 100% correct with their information.

Thanks for posting this! DH has UHC/Empire..he knew nothing about this. I just called him at work and he is going to fill out the paperwork and mail it today.

We recieved ours as well. Def. not as much as we thought, but can still consider it found money..
