FMLA paperwork
Did your employer ask you to fill out paperwork prior to your delivery date? And did your doctor's office charge you to fill out the paperwork?
ETA:
If you filled it out ahead of time, did your dr have to fill out more paperwork once you had the baby?

Yes, I filled out paperwork before going on Maternity leave. No, my doctor did not charge to fill out the paperwork.

My employer had STD insurance. The only form I gave my Dr for it was a release so they could give info to the insurer. I gave all my info over the phone to the disability insurer and they contacted my Dr.

Yes I filled it all out prior and no my dr didn't charge me for filling anything out.

Yes, but I didn't have to do it ahead of time. I think I had a week after the birth to fill it out. And no charges.

Yes. For my first all of my paperwork had to be in prior to taking my first day of leave.
No, the doc didn't charge.
