GHI - city of NY employees (CBP network)

Does anyone know what IUI / IVF coverage there is ? From what I can gather they will cover 3 cycles of IVF fresh or frozen. I can't find anything about IUI's though. Also, perscriptions are covered through express-scripts. When I go to the website to "price a drug" they say that NOTHING is covered although someone had told me that some of it was.

TIA for any information ! I am trying to gather as much info as I can. I am planning on making an appt. with an RE soon, but want to know what's covered before I go in to make sure we're on the same page !!

From what I remember, IVF is three cycles covered at 75% and you have to be between certain ages, I think something like 25-42, I can't remember that part. There is no limit to how many IUI's you can do. They cover them at 100%, you have to pay the co-pay. And sometimes it is a double copay for the office visit ($15) and then b/w and sono are considered another copay ($15). I just got a bill from my dr and fought with GHI about it yesterday. The drugs are through express-script and you have to meet a $100 deductible. I was on clomid which I got from CVS and wasn't covered through CVS so it cost me $25 and Ovidrel, which I got through express-script and cost me $75. I had one successful IUI, so I don't know about all the IVF meds

I believe this is correct! Also depends what doctor you go to - if they accept the coverage.

i also got the clomid thru cvs i dont remember it being $25.00 - i think it was cheaper. not sure. as far as the ovidrel it is an injectable and you have to meet ur deductable of $100 for the year plus a copayment. I forgot how much the ovidrel was but once i reached the $100 deductable i had to pay just a copay of other injectable meds (lovenox is 25 a month, heparin is 10 a month etc) i went thru freedom fertility and thru curascript (express scripts for injectable meds) Im not sure of other fertility injectables im sure someone on the board can help u out there.


as far as the iuis - they are unlimited if ur btwn a certian age. with ivf u have to have pay 25% of GHI's fee and its covered 75%. You have 3 attempts. i forgot if its combined frozen or fresh - i would have ur re office call them and find out and you should double check with them as well (call ur insurance company and just verify what ur drs office says)
