Giving Authorization to Relative For Medical Treatment of Child...

DH and I are going to Key West this weekend...by ourselves!!!Chat Icon Chat Icon Chat Icon
First time away from Jordan. He will be with my mom. She would like us to write a statement allowing her to make medical decisions, authorize treatment, etc - gd forbid.

Is there something in particular I need to write or I can just make up a basic statement?

I did this a few times when I went away without DD. I just typed out a statement had her medical insurance info on there and had my signature notarized. I also left the actual medical cards.

I did this a few times when I went away without DD. I just typed out a statement had her medical insurance info on there and had my signature notarized. I also left the actual medical cards.



I did the same, only I didn't get it notarized. My ped. said it wasn't necessary.

Call your doctor and ask what they require. I think they might all have their own "rules."
