HELP! NYS Short Term Disability Paperwork

Ok so Ive had nothing but trouble with my HR person since I went out on maternity leave. She did not explain my benefits correctly, and said she "doesnt know" what papers I need to fill out for NYS STD because she's in MA.

I was under the impression that I sent her the correct forms to fill out, but three weeks later she sent them back saying theyre only for Worker's Comp (which they arent!!!). Because it took her SOOOO long to send them back to me im afraid its too late to file.

How do I find out if these are the correct forms, or does anyone know which forms are correct?

With my company I let them know I was going out on maternity leave and they gave me the number for the liason who handles the short term disability - for me it was Aetna. I called them and they sent me the paperwork to be filled out. Maybe your company is set up the same way? HR should at least have a number for you! You only have a certain amount of time from going out to fill out the paperwork.

I sent in the right papers weeks ago but my HR person thought they were workers comp papers so she sent them back to me not filled out

Im going to kill someone
Im currently writing an email blasting her, and sending it to everyone above her

so sorry you are going thru this!! My disability was thru Hartford insurance and they were great
