well duvet is a cover to a comforter. you can buy a plain comforter (down usually) and you buy duvet covers with different designs and put the comforter inside the duvet.

a comforter is a heavier blanket and i think a bedspread is much lighter, sometimes just used for decoration on a bed.

duvet is basically a giant pillow case that people usually have over a down comforter to protect it (they are PITA to wash) You can also put a duvet over an old/ugly comforter for a fresh look.
Comforter is filled with that fluffy poly-fill material or can be down (feathers).
Bedspreads are purely decorative. They are often a kinda stiff material & not exactly something you would cuddle with. They don't have any "filling" They look very nice but most people roll them down for sleeping.

Comforter: Made with down feather or symthetic fill. Got to side of your bed. Think if it as a "down blanket".

Duvet: Term sometimes used for comforter. Think thick and fluffy. A true duvet does not come down on the sides. It goes on the very top of your bed for extra warmth. Duvet cover is essentially a comforter cover.

Bedspread: very thin. Think thin quilt but not with the quilt style. Usually decorative that goes on top of your bed.
