How do you fill out these Disability forms?

How do you fill out these Disability forms?
HR department wants me to fill out the forms now, so they have on file, but it asks for my date of leave, when I was hospitalized etc.. How can I possibly know when these things will happen? What if I go into labor this weekend? My Dr has to fill out the portion on the hospitalization - he is not going to be able predict when I'll be hospitalized?
Has anyone else filled out these forms?

Just fill out what you can. Once you have the baby, they will have you provide the rest (including the part that the doc has to fill out)
