I have for when I worked at a health care facility and for law enforcement. Obviously the LE physical required a bit more (hair analysis, EKG, etc.).
For the health care facility my primary asked me basic questions, took my BP, temp and I believe they did some basic blood tests (CBC, BMP, etc.) which obviously requires a blood draw.
Too bad you didn't ask this question last night! I just threw out all my physicals.

Mine was just a general exam and the only test was a urine test.

In college I worked for memorial sloan kettering and they took 4 vials of blood and did a general physical exam. For my current position with a different company I had a medical interview (just physical history) and had a urine test.
