Intralipids questions

Sirm put me in intralipids every 3 weeks until 20 weeks. They never rechecked nk cells. Kofinas mentioned possibly changing to every 2 weeks. I thought he was checking nk but he says he doesn't check that. He said he would only up frequency based on how babies were doing and they were doing great so stay on every 3 weeks. Does anyone know why kofinas uses intralipids then if not for nk?

So here is my issue- I have terrible veins. I always get stuck the max times of 4 and it's really stressful and painful. According to sirms protocol should have gotten it today (stuck 4 times but didn't work) and 2 more times until 20 weeks.

Now I know dr b does it every 6 weeks until just 12 weeks.

I'm going to ask kofinas if i can stop when I see him thurs but I'm wondering if I still need them or if I need my nk cells rechecked (which I'm not sure they will do).

I dont really know why he'd do anything without repeating the immune testing. Im not going to pretend I know everything about intralipids cuz I DONT but isnt that the whole point? To lower NK? How else would you tell what your NK levels are at if you dont do the immune testing? What's annoying about it is it's EXPENSIVE, but you do what you gotta do.

And Dr. B doesnt do it every 6 weeks standard. He does the repeat immune testing after each intralipids until your levels are normal. My last IL was 2 weeks ago and I have to get another round done asap cuz they were still high.

Yeah I find it weird that Sirm never retested. They just went off my original tests. If it's just nk I can just have it done at quest and it costs me nothing. They don't do everything reprosource does though.

Then why does kofinas even use them if he doesn't test nk?? These drs are so odd!

Kofinas mentioned doing them for me once, I forget why but I don't believe it was immune related bc Dr.b wasnt even prescribing intralipids for me. Maybe its for something else? Just ask! also Dr.b did my immune testing everytwo weeks to follow.

You can test nk at quest?!

Yeah some of the immune panel can be done at quest. It might just be nk though.

Yeah I'll ask. He never said why. Just that he prescribes based in how the babies are doing. Mine were ok so he kept it at 3 weeks. I'm really hoping I can be done but don't want to stop without knowing how nk is.

i would def ask your RE to repeat the immune profile, it doesnt make sense not to.

i did every 4 weeks til 12w with dr. B and that was based on my nk test coming back normal after the first round of intralipids prior to ER that cycle.

I called kofinas office and he uses it for growth. They do test for immune though so I can do bloodwork there. Sounds like it's a reprosource type place but in Chicago. I hope ins covers this one!! I have to talk to him at my appointment.

Like if the baby is growing properly. Not sure what it does to help the baby but apparently it does. My babies are fat enough. lol
