Medical Release for Vacationing Parents
My mom will be watching my kids when we go on vacation and I need to leave a note stating that she has permission to take them to the doctor or hospital in case of emergency.
Is there some type of form I can print out for this or should I just write something up?

We wrote something out listing dates & such, & actually spoke with our pediatrician about it.
If I can remember correctly, & this is in NC, but she told us an ER will treat life threatening issues without our concent, the problem is in the minor things. So we signed something in the ped office.
I also gave my in-laws a sealed envelop with all necessary information regarding accounts/documents, etc.  It was thankfully returned sealed.

Ours is also notarized.
I would make copy of your health insurance cards as well.
We also have a note in our kids charts with the ped so they know its okay.

That's almost identical to the one we used. And definitely have it notarized (my ped wouldn't accept it without being notarized).
