My RE Appointment - long...UPDATED at bottom - spoke with finance & now the NURSE - and learned something new!
Okay - first let me say - i'm sorry this is so long...but i'm a very detail oriented person and always enjoy more info...
We met with Dr. Boehrer at RMA of NJ... He's very low-key, very soft spoken... He asked some questions, based on the MOUNDS of paperwork i had to fill out and then asked some questions about my cycle. I had my charts to show him, which showed that i have been ovulating and that my cycle has been, in his word, 'weirdish'. I told him about the B6 and he kinda smirked...saying it helps, but not enough really. He told me all the stuff that I already knew (about testing, etc)....and said what to expect....i'd tell you here - but then it would be silly to just tell you again as it happened...
First thing was the ultrasound..... This is where the biggest OW EVER comes into play. He first checked my cervix...i guess to make sure it was kosher. End result - it's fine. Did this part hut? Not too much - just uncomfortable.
The ultrasound was pure, unadulterated H E L L... Most of you know I cramp when i'm ovulating, so i already had little cramps. The internal sonogram hurt a LOT!!!!!!!!!!!!!!!!!! He found my right ovary within minutes and that was pain on a 5'ish...which sucked, but deep breathing and focus helped with that. Then he noticed a fibroid. He was able to determine that i don't have PCOS because the numbers of follicles i have isn't immense (apparently, if you have PCOS, you tend to have last months' follicles still there, so your numbers would be HIGH - mine were about 6-7, which is within the norm). He then started to try to find my other ovary. This process went on for about 10 minutes of pushing and prodding on my stomach and internally. This HURT LIKE DEATH!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!! He kept asking if i was okay and i said yes, because what on earth was i going to say???? It hurt a LOT. My left ovary was apparently higher up, because as he said nicely, in women who are a little overweight (God bless him), some ovaries like to ride up...he eventually found it. It didn't have as many follicles..
After I got dressed and got weighed by the nurse (if the sonogram didn't hurt, that sure as heck did)...and then went into the waiting room to wait for the nurse. She had to get some info together. We waited all of 10 minutes maybe and were whisked back into a room to have a consultation. The plan is that he is concerned about the QUALITY of the eggs that i'm producing, based on the sonogram...so he's doing something called a clomapine challenge.
So - here's the plan Stan...
Day 1 of my next period - i call them and make an appointment for day 3 blood work & an HSG (this is a procedure done at a radiologists office, they basically put die into you through a catheter and make sure your tubes are not blocked...among other things). This has to be done between day 6 & 10 of the cycle.
Day 5 - I start taking Clomid (which basically tricks your body into thinking you're menopausal)....this will be part of the testing.
Day 10 - i stop taking clomid and go back in for more bloodwork. If there is something wrong - my numbers will be HIGH, if everything is fine, my numbers will be low. They will call me with the results by the end of the day. One thing i LOVE is that they will NEVER EVER leave for the day until every single person who had blood work done gets a call...
Day 10 - i have to go back in for more blood work to be done to check on levels of stuff.
Day 21 - go back to the office for more blood work.
During all of this fun stuff - Dave has to go in ONCE. He has to have a Semen Analysis done (they have a whole room for the boys - movies, magazines, etc)...and have some blood work done as well.
The nurse told me that there is some concern about the fibroid. She said MANY women have them, and in some women they aren't an issue at all. BUT - for some, the fibroid keeps implantation from happening and in some cases, will ultimately cause a miscarriage..so once all the first cycle testing is done, that will decide what happens then.
The fact that I don't BLEED outwardly is a slight cause for concern as well. When I told the doc and the nurse that I could wear a light days pad for the entire time of my period and never have to change it seemed to make them twitch a little. The nurse said that could be normal for me, or it could be that i have a problem with my uterine lining... Nothing that more testing won't reveal...
Overall - i'm very happy with how it went. I'm in a LOT of pain - because I guess the sonogram combined with the cramps from ovulation.... I'm nervous about the Clomid - they told me that it really messes with you... They told me that charting/temping wasn't necessary anymore because they're on the case now. He appreciated my charts/temps that I showed him, because they showed him a clear pattern of a 'weirdish' cycle...
This got real long, real quick... :) I'm sure there's a lot more to share - but it's all still kinda sinking in. Tomorrow - i have a phone call with the financial office and then the RN calls me to see if i have any questions. She said most people, the day after find themselves with 800 questions, so they started calling their patients that day to answer any ?? they may have. I really like the people in the office - they are all wonderful!
I cannot wait to meet everyone at the GTG!!!!!!

sorry it was painful.. but it seems like your dr is very proactive and is going to do a lot of testing!!!!!
if you dont mind me asking how long have you been trying before consulting an re?

If you had PCOS, you would be able to tell because your ovaries would look like swiss cheese on the sono. That is what mine look like.
The fact that the sono was so painful for you makes me . Is it painful at all for you to BD? Did they mention that you could have Endometriosis? They cannot diagnose this without doing a small surgery, but based on you pain and that you aren't bleeding, i'm kind of being led to that. (But of course I'm not a doctor). I have endo and intercourse is usually uncomfortable. It used to be extremely painful, though.
Be prepared for some side effects from the Clomid. Most occur AFTER you finish the pills, for about 2 weeks after. The mood swings are awful, and I had bad dizziness while taking them.

He didn't say anything about endometriosis - and he knew i was in pain. I really think it had to do with for me normally, around O time - i'm rather uncomfortable. I normally have cramps and sometimes have had to TAKE something for them.....so to add to that a search & rescue mission for my ovary....maybe that was with all the pain. I'm 100% going to talk to the nurse that calls me tomorrow about it.
Intercourse isn't normally painful for me...but um...that first moment, when dave, um, goes on in...that hurts a little...i've always chalked that up to him being, um, a big boy....

Just spoke with the financial lady at the RE office. There are NO stipulations that i have to be 'diagnosed' infertile before I see the doctor. I am cleared to start. She actually said the way the doctor wrote up my paperwork, that even if there was a pre-requisite, i'd be fine, because he's concerned because of the fibroid and age.
Will update again after I speak with the nurse about the pain yesterday! (i'm much better today)

I meant to answer this - i'm so sorry I forgot!!!!
We've been trying for 6 months. The reason I went to the doc about it though is that my LP is short. I spot on day 9 or 10. That, in addition to my age and history (i have a LOT of medical issues), made me worried.
My doc originally told me a year, which I was fine with. Then - after temping/charting for 2 months, I noticed the short LP and went back with that info. The doc then told me, with a PROBLEM & my age, 6 months is my timeframe.

just hung up with the nurse....
The pain is normal - when they have to 'search out' an egg - the 'struggle' can cause excess pain. She said most women don't ever complain - but she'll add the note to my file. I also mentioned that i had a tiny bit of spotting and she said normal, when 'struggling' to locate an ovary.
I don't have to continue to take the b6 vitamins to extend the LP - because it wasn't doing much. She said it can't hurt but it's not needed.
SOMETHING NEW - the doctor wants to make 100% sure that i AM ovulating at all. I have to go on September 24th - which will be day 21 and have blood work done. If i AM ovulating - my progesterone levels will be expected to be at a certain number...and if i'm not - they won't be and that would affect the future stuff...so we'll see.
ANOTHER NEW THING - i have to pay a copay every time i get blood work done...that STINKS.... my co-pay for specialists is $30 bucks...so it looks like financially this may hurt a bit at first - but i KNOW that others don't have good or any coverage - so i'll shut up about that now.

another great update !!! glad to hear things are moving along wonderfully! so the nurse said the pain is normal..thats great! i hope you are feeling better though
