I used her party dress. I also bought a personalized onesie on ebay ($10.00) and used a Tutu (30.00) with it.
I think the accessories help too. The letters that spell out "ONE" and balloons. Simple things...

My photog allows 3 outfits and the cake smash. But I'm not sure I'll be suing all of them.
Tutu dress
Lace romper with matching fancy headband and pearls
Bathing suit with sunglasses and beach pail (she's a summer baby born right by 4th of July)
Diaper cover for cake smash
