NYS disability/maternity leave
Im trying to get things in order for my maternity leave...
How do I go about getting the paper work for NYS disability (that standard 150/week or whatever it is...)
There is no HR department at my job, and the owner has no clue...TIA

Just call the company you pay your disability to for the company and ask for a form. They will fax it over and you just fill it out and have your boss sign it. I was the HR department too. Then you just fill it out and send it back. Their is certain time frame and I think I mailed it out from home when I had. I also think my Dr. had to sign it.
ETA: When I received my payments, it was one then then the second was a lump sum at the end of when I was suppose to receive it. They also hold one week. So a v-birthday is 6 weeks but you get paid for 5 and a c-section is 8 weeks and get paid for 7.

You can print them from the NYS Workers comp website.

Well, for that I would do what Adrianna said and call the disability company and see what they tell you in regards to NYS Disability as well

Not through my employer- I pay into a private company...
Yes its more... but Im basically getting back the money I have paid the past 2 yrs...
So I was hoping to recieve both

Like Aflac? If its like That, yes you should be able to get both, sicne its not corporate related at all.

Yes, you can receive both DI payments as long as the private DI is for short term disability.

Yep- its a private company just like aflac (not sure of which company) and it is specifically for STD

You will get paid from your private STD company plus NYS disability (which you pay into with each paycheck). I got both when I was out with Marron. It ended up being very close to my whole take home pay when both amounts were combined.
Since there is no HR dept at your job, call the insurance company that carries the disability insurance for your employer and they will tell you what to do. As far as your private STD call them and ask them what to do. You will have to have everything filled out twice -- for each separate company.
This is NOT worker's compensation. It is disability. Worker's comp is for when a person gets injured on the job.
