One more passport question... last one- promise!
My passport expires in May AND I need to have my name changed on it-- what catergory does this fit into??
I am looking on the passport site-- and can't seem to find these two instances together.

**I Think*** if you send it in now, you can just do a name change. They are re-issuing it, so you would get a several years on the new passport.

I believe it's the DS-82. That's the form I used when I renewed my passport and changed my name to my married name. I also had to send in an original copy of my marriage certificate.
