Q about CPSE Paperwork
I just got the packet in the mail yesterday, I noticed there is a "Medicaid Consent Form". Is this something you filled out or can you leave it blank? Is it sort of like billing insurance? TIA.

I brought it blank to my first appointment, but they told me to sign it.

It's so the school or agency can bill Medicaid for reimbursement if the service is eligible.
