How about...
Having sex in the borrowed car when you were told not to?
Seriously, we had to TELL FH's friend not to have sex while he was borrowing our car. And he definitely didn't listen.

It's really a toss up between the BO and smoking for me. The other options, though annoying aren't as bad IMO.
ETA: I'd almost prefer the seat and mirrors to be moved that way I know they adjusted everything so they could drive safely!

I put smoke in the car presuming the car owner is a non-smoker other wise lingering BO would be my second choice.
I expect people to move the mirrors & seat when they use my car - they can't not and it would be difficult to put it back exactly how I have them. I don't care if people eat or drink in the car - I'd like them to be careful but accidents happen and it's not something I don't already do anyway.

I would say smoking. BO - while pretty bad - may not be something that someone is aware of.
In order
Smoking is an intentional offense that lingers.
Leaving the tank empty - if someone lends you a car, filll it up. It's common courtesy.
Eating is bad but still more understandable.
The seats & mirrors I don't mind.

DH smokes in my car and it drives me nuts! I hate it!
Funny thing about moving the seats and mirrors - my dad borrows my car a lot on the weekends. He never wants to move my mirrors or seats bc he doesnt want me to "forget to adjust them" 
So he squeezes his fat belly into the seat and literally drives with his belly touching the wheel!! I always tell him to move the seat back but he never does!

Smoking. If the person is a non-smoker especially. It's hard to get the scent of smoke out of a car, clothes, etc.

I chose change the radio preset because that is just a PIA to fix and redo. Smoke and BO are a close second though.

I think they are all awful, but I picked changing the radio presets. This suggests a form of arrogance and entitlement that is in a league of its own, IMO.

I have to say smoke. DW and I both aren't smokers. If someone borrow our cars and smoked in them..I would probably backover them when I got the car back.
