Rehabilitation facility and nursing homes
Hi I am hoping someone can help me. Long story short my dad needs to go into short term rehab to deal with medical issues (he's 77, has limited mobility, and other health issues one is weight related). He's in the hospital now b/c of stomach virus and dehydration. However he's better on that front. His dr (who knows he's had the other problems, we all tried to get him to lose weight and other things he's got to do for his health) is keeping him in the hosp to get PT evaluated so he can go into a rehab to get diet and weight issues looked after and more PT on his legs. I was told look into Long Beach Rehab but my cousin just rang and said no Daleview is excellent, he had his mom (my dad's cousin) go there after her stroke and it was the only place where he felt didn't smell bad, patients weren't "put in corners and ignored" - the staff was very interactive, etc.
Anyone have experience with either places or recommend another rehab?
There's also a small chance if the rehab can do no more for him and he really needs to move into a nursing home or assisted living (I'm going to push for that if it comes to it) - I would want him to be close to where he lives now b/c we are moving into his house and my sister lives nearby. That would be around Lynbrook/RVC/O'side/Baldwin area. There's a place called Altria on Penninsula Blvd (I think it's the new one?) anyone know anything about that? What about Sunrise Senior Living on Franklin Ave? (In Nassau Co)? I searched posts and saw a few people liked a place in Huntington - can someone tell me the name?
Lastly, the social worker at the hosp that my sister talked to sounds like she couldn't care less about helping us. Is it because they don't want to do anything for the patient if there's family available? I thought these people were there FOR the patient AND the family. Not like I've done this before you know? She basically told my sister not up to the dr to get him into rehab, depends on the PT eval (which is going on today) and the insurance. I know that the ins will be a pain to deal with but is there anyone that can help us with forms, etc? Like a third party? would I have to call an attorney at this point? Any guidance/advice/tips would be really helpful! Thanks! Mary

Aww well first off a lot of for you cause i know how hard it is to deal with this kind of stuff, I've been doing it with mom for years. I don't know of any places in nassau but the place everyone raves about in Huntington Is Carillons nursing heres the link Link theres other theres Sunrise, but I don't know anything about it. I have friends who are very good friends with the owners of carillons, and I had tried to get my mom in there, but she didn't meet the criteria, she needed a private room, and they couldn't do that. But I've been there many times, the place is always so spotless, and clean, doesn't smell or feel like a nursing home. The one my mom did get intot hat was very nice was Huntington Hills heres there link Link they are probally easier to get to, there on the south service road of the lie. they had such great programs and interactive stuff for everyone! I def reccomend them. Good luck in your search and i hope your father does well where ever he goes.

I don't know much about nursing homes or assisted living but for a rehab, North Shore was great. My grandfather was there after he broke his hip and they were wonderful.

Go to the NYS DOh website. Scroll to the right and look up Nursing facilities. YOu can review the latestest DOH surveys. that should give you an idea of how the facilitites are doing. FM me with any questions.

I don't know about the ones you mentioned but my mom is an RN at Sun Harbor Nursing Home and Rehabilitation Center in Roslyn. The patients there get excellent care. Good luck!

I dont know if you are looking for north shore or south shore facilities. We have had our parents at the Center for Extended Care at North Shore Hospital and at Glen Cove Hospital for different types of medical rehab and they were both cared for very well. Glen Cove is a little off the beat and path, but a good facility.

I'm a medical social worker and I hope I can be of some help to you. Where do you want him to be in Nassau or Suffolk. The other thin is the SW is right, rehab needs is based on a PT eval. Once they make their recommendations a PRI must be completed by an RN and then it is submitted to Nursing facilities. I really like St James Plaza and Island Rehab. St Charles also has a phenomenal rehab program. The SW should be coordinating the transfer for you, if our dad has Medicare (which he should @ 77) medicre will pay for the 1st 90 days (which should be plenty to cover restorative services). If he doen't use the full medicare coVerd time he can apply it to homecare services which will include home PT about 2-3 time per week (this will get him home sooner). He can also go to ADHC whch is adult day care whch also has PT, so he can come home in the evenings. I hop this was helpful. What hospital is he at?
Most reab facilities are also Nuring home, so if he needed o stay, he could just transition to the Long term unit

Hi, THANKS SO MUCH - I talked to the SW before and she explained a bit more to me. When talking to my sister, either she was dismal in her hopes that dad could be put into an acute rehab. She did'nt tell my sister (or my sister didn't listen or tell me) that the reasoning behind that is that acute is PT 3x a day and my dad uses oxygen all the time, and is mostly sedentery so 3x day may be too much for him. I personally don't think so but she could be right.
But sub-acute facilities - this is where the SW thinks he may wind up being put into - are they just in nursing homes?? We really don't want him in a nursing home for rehab. We feel he will see the people there and get more depressed and think that we are "putting him into a home" which we're not but being at his age I think he may feel we are (memory isn't the best right now w/ him either).
Also thanks for the info re: medicare - he does have that plus he kept private insurance through his job after he retired so that helps out alot too. Going to also FM you. THANKS! Mary

Tahnks i did but sometimes a person who has experience with the place also helps...surveys can only tell you on one day how the place is you know?? But I have looked at the site, it's really helpful.

Just wanted to say thanks to all that replied and FM'd. We found a *great* place, it's very clean, not over crowded, and it's a mix of long term elderly residents and short-term people who are all ages, there's a few in there in their 50's who are getting PT for hips or knees from replacement surgeries. Dad watched the Giants game with 2 of them yesterday. LOL Thanks all.. Mary

I wish you lots of luck. We are going through this with my Grandma. It is so hard to send a loved one to one of these places. She is in one right now in Queens that is horrible!
I I hope his stay is short
