spinoff to babies getting a vaccine twice by mistake
how does your ped keep track of vaccinations?
it's all computerized at my peds office. at every appointment i get an updated printout of ds' vaccination record, and they put a new one in his chart. so there's really no way to make a mistake.
i took it for granted that this is how it's generally done, but i guess it's not?

At ours they write it in the chart and then in a shot record book they gave me at their first visit. I'm all over them about my book, so I know that's up to date.

No, that's great that they do that. It's just written down by the nurse in the records. I *try* to write down the date when I get home on her immunizations chart, for my reference, and try to be aware of what she SHOULD be getting at each appointment.
I'm guessing they put it into the computer, but have no idea.

My ped's office is also computerized and once we get into the room DS's chart is brought up on the screen and they mark his record card that I carry with me.

mine wasn't given by mistake...I was told that it was no big deal for him to have an extra hep b shot in him.
He received the first in the hospital(which I will NEVER do again)...
then at his 2 month visit, I was told they ordered new vaccines and it has hep B in it, their shot schedule is going to change, so he will get this new one, resulting in an extra hep B.
I questioned it, they said its happening to a lot of babies and its okay.
That is when I started doing my own research on shots.
I don't think they are on a computer system, I have no idea though.
With the pox shot last week, she did nto bother checking his charts...just assumed since I put off MMR, that I put off that one as well.
ETA: I have them fill out his book each visit

I just posted this on the bottome of Ang's post:
My ped. has a seperate page in his folder that lists all the vaccines and the ones DS has received. Everytime DS receives a vaccine my ped. signs next to the vaccine and than either myself or DH has to sign as well showing we not only approve that he will be receiving the vaccines, but that it was given to him.
I thought this was standard practice with all doctors. My ped. is also a solo practitioner so i dont know if that has anything to do with it being he is never seeing a different dr...it's always just him.
ETA: I also have a vaccine book that my ped. gives me and he has me write down in my book what date DS received the vaccines.

Only the pediatrician hand writes all vaccinations on the chart before he gives them to my DS and then he fills out my immuniziation card that was given to me at the hospital.

My ped writes it into her chart on before he goes and gets the shot to administer - plus he always writes it into he immunization book he gave us when he records her weight and height when we sit down with him after each visit.

My ds had the hep b shot given to him an extra time by mistake. (we are no longer with that dr., they made more then just that mistake) At that dr's office they would write it in the book that I kept then 2 places in the childs chart however after it was already done I noticed it in my book that the nurse crossed out 1 of the dates it was given and put the date that she just gave it to him. Well when I spoke to the dr they really didn't care that she crossed it out but they did say it was not written in the chart in one of the spots it should of been. But they still didn't care they just said don't worry nothing can come from it

My pediatrician is supposed to record it in the child's file and in the computer system.
<long pause>
But what worked best for me was that I had a chart he gave me in the beginning with what shot was given when and I followed it myself - checking off each one that my son received. I also wrote in the appt date.
I have higher expections going forward especially reading this thread and seeing how organized some other offices are.
