Say overnight when the ferries don't run?
How do you get to a hospital? Do they have one there?

I would imagine they have a boat with medical personnel that would take you to the mainland. I can check with our friends who have a house there.
Hospitals/medical centers near Shelter Island: SOUTHAMPTON HOSPITAL (about 13 miles; SOUTHAMPTON, NY) EASTERN LONG ISLAND HOSPITAL (about 24 miles; GREENPORT, NY) CENTRAL SUFFOLK HOSPITAL (about 24 miles; RIVERHEAD, NY)

My friend lives on Shelter Island and I believe there are always emergency boats on standby in case of emergencies.
They have a small hospital too I think.

Re: What happens if you have a serious medical emerg on Shelter Island?
They are able to get someone to operate the ferry in emergency situations. They also have the ability to air-lift off the island if it's something extremely urgent. Eastern Long Island hospital is on the other side of the North ferry so it's a short ride once they get on the ferry. There is a clinic on the island but I don't think it's 24 hours (I do think there is a doctor on call though). I actually think being in Montauk is more remote then being on Shelter Island!! It takes a LONG time to get to the hospital from there.

we were just in Montauk last month, and I was thinking this. If someone needed to get to a hospital, it would take a while!

I'm sure if it was really awful they'd get a helicopter.
