We needed to send in a copy of our deed, and a copy of our driver's licenses with our new addresses on them. Whatever town you are moving to you can go to the website and download the STAR application. We downloaded ours from the Town of Oyster Bay's website.

Town of Hempstead told me we needed a copy of our Title, driver's license with current home address and/or vehicle registration with current home address.

Town of Islip all I needed was a copy of my deed and the star application. It really depends on the town.
