ugh I am so sorry that happened...I would definitely talk to her and see when she says but if she plays dumb, yes, I would look for a new therapist 
eta - after re-reading your post, I think that you SHOULD fire her, there have been more than 1 "incident" and I would not be comfortable with that

My gut is telling me GET RID OF HER! I don't like the scratch, it looks like a nail scratch as if she slapped him in the face. Fire her and then report her. GUT rules out how good she MAY be. JMHO!!!

I would talk to her about he scratch and ask her what happened. If your not satisfied with her answer than maybe you can sit in on her therapy until you find a replacement? Would that be too distracting for your DS? So sorry this happened!

ITA with everyone. I would definitely find a new therapist asap. Something is telling you that there is something off or wrong with this therapist. I would probably report her as well. You should call the agency and let them know what happened and see how they want to proceed. If the therapist came right out and told you that DS hurt himself by falling, bumping into something, etc., it would be a little different. But the fact that she denies that anything happened and that your DS now has a bruise and scratch is VERY shady to me. I would address this now to give yourself peace of mind. And regardless if this therapist's excuse, I would find someone new. I'm sorry you and your DS are going through this .

You mentioned she said "See you tomorrow at the meeting"...Whatever meeting this is, this definitley needs to be discussed!!

That therapist is our team leader. The meeting is the monthly team meeting with all the other therapists too.

I wanted to add that I don't think she hurt DS. But I feel she is hiding that he got hurt for whatever reason.
If he got hurt. Just tell me.
Don't tell me he crushed his own finger when I KNOW he can't do it. Don't tell me nothing happened if I see a bruise on his face that was not there this morning.
He did have another therapist before her this morning. But again, he did not cry with that one and there was no sign of a scratch.
I can't prove anything. But I am very suspicious because it seems to happen only in her sessions.

as a therapist, i would absolutley tell a parent if something happened during my session. that being said, that scratch could be from a ring or a finger. i have done that plenty with hannah when i have held her face or tried to prompt her myself. you need to immediately talk to your team leader, who should be understanding to your concerns. i hope it works out
etd to add, maybe you can watch or videotape her sessions?
