Did you get a tax form from disability?
I was waiting for one from my maternity leave but it hasn't come yet. I really want to get these taxes done I heard I might actually get money back.

No. My employer actually redid my W2 to include my disability.

It depends. I was on STD for 6 months and collected Aflac. I collected the max NYSD and my employer did not pay any part of my salary. I do not have to report it as income. This is why Aflac takes the money out after taxes not before (like a 401k or cafeteria plan).

i got my disability from my union, separate from my paycheck and i got a W2.

I get no pay from my company and I am getting disability payments from an insurance company....I never received a W-2 either. I only got one check for 2009 (had the baby 12/13) so I am wondering if this is taxable too. Should I be getting a W-2? Who do I contact about that - my employer? so confusing!

I am going to call my disability company today bc I never even thought of it. I was getting a separate check from them the entire time I was on disability.

yes, but my disability was paid through a separate company, I got a w-2 from them with my maternity payments
