How do introduce solids to my 4 month old?

I know i should know this already considering i already have a 2 year but he is the worst eater and i want to make sure i get this right now. My confusion is we already put rice and oatmeal in bottle all day because he has severe reflux and is on hydralyzed formula (elecare). We were given the go ahead to start this month. I tried tonight just making a bottle with no rice in it and instead put the tablespoon of rice in bowl and mixed with formula. i felt like more was comming out then going in. but how many times a day do i do this should i stop doin rice in all bottles? does it matter if i do breakfast or dinner. IM SO CONSFUSED

It takes a little while for them to get use to the spoon, so in the beginning more really does come out of their mouth than goes in. My ped wanted me to feed dd at night to help keep her full at night in hopes to help her sttn. I don't think it really matters though.
