My insurance covered it. My peds office wrote a letter of medical necessity and took care of everything with the insurance company. The insurance company then gave me a list of home care supply providers that shipped me monthly supplies.

Same as PP.
It gets delivered to my house. It was a bit of a fight though b/c every time I called the insurance to ask them about it they told me something different. And I ended up having to buy it myself and submit for reimbursement while it was being figured out.
I also requested the powered to the RTF and that delayed things a bit, but it all ended up working out fine.

We get it covered through our Rx coverage through my husbands union (UFT) and we got the same as PP (letter of med. necessity) plus an Rx from the dr. We pick up 5 cases of RTF bottles every month at our local pharmacy.

mine didnt. I had UHC PPO and Medco pharmacy. b/c it was over the counter they would not. i appealed, had dr. write letters, no go.
every company is different.

I have BCBS PPO and it covers 85% of your formula receipts after a $150 deductible.
ETA: Insurance would only cover it if the pediatrician documented that we tried all other alternative before using alimentum (ex: regular, soy then alimentum).

We have BCBS PPO and they cover 100%. When I first started asking they said 75% then the next time it was 80% and eventually they said 100%.

Yes, my DS has a milk protein allergy and the specialist prescribed it. We have empire and it covers it. They send it to my house.
