Extension/dormer
We're in the beginning phases of possibly doing an extension/renovation/dormer and aren't sure what to expect as far as cost (for architects/permits/contractors - everything.) We would be adding a two story extension and reconfiguring the floorplan to have 4 bedrooms and 2.5 baths (from 3 and 1.5). Basically I'm looking for btdt advise, experience, cost and even contractor recommendations. I want to avoid making big mistakes that others have made so I appreciate info about any and all experiences.

You should expect to pay about 130-140 per square foot which includes everything. Depending on finishes. This will give you a pretty good starting point.
You could call Kadlec Construction. They are in Port Jeff. The owners name is George. His # is 631-331-1303 or 631-331-3383.
