Go to the town hall in the town that you were born in. If that's not possible, I'd call a town hall and ask what to do. I got my new o e from the town hall. Gl

I just did this, and went right to the Town of Oyster Bay town hall, walked in, filled out a form, showed them my license and $10 and 5 minutes later, had a new copy.
