Maternity Leave
I am due in a few weeks and plan to take my 6 weeks maternity leave plus an additional 6 weeks through FMLA. For the OP-198 form - does my doctor need to sign it? What about the FMLA form? (I am not scheduled to have a C-section and baby is head down so it is unlikely that I will need one.)

I'd check in with your HR dept. though I'm sure there are other DOE teachers on here that can help! It's been so long I don't even remember what the doc signed for DS#1. But my agency's HR dept provided the forms to me.
